FAQ MODULE

- please write an email saying that you want to study at our Centre to the following email address: sjpdc@uni.lodz.pl   

- in a reply to your email you will receive a registration form to fill in and a list of required documents (attestation, apostille, medical certificate, passport - photo page and parental statements if you are a minor) 

- you need to fill in the form (with exact data as in your passport, the choice of the type of course, the direction of future studies, the name of a Polish Consulate where you will apply for a visa), 

- then please send the scans of the form and the required documents to sjpdc@uni.lodz.pl   

- in response you will receive a letter (in English) confirming preliminary approval for admission and bank details for payment of the course fee 

- then you should pay the money and send a scan of the payment receipt to sjpdc@uni.lodz.pl   

- after we receive the confirmation from the Finance Department of the University of Lodz that the payment is in the university bank account we will send you:

            - a letter (in Polish) addressed to the Embassy of the Republic of Poland 

            - a scan of the letter confirming the booking of accommodation in the dormitory of the University of Lodz

In order to receive a student visa you should go to the Consulate with both documents.

Upon arrival: 

- you come to the Centre between 8 am and 2 pm to the room 2.10 in the building at 16/18 Kopcińskiego Street 

- you should have original documents (matriculation certificate, legalization of matriculation certificate (ask for it at the Embassy or the Ministry of Education), parents' statement (if you are a minor), health certificate (that there are no contraindications to study), 2 photos, 60-70 PLN for student insurance PZU NNW 

- you report to the dormitory: Lodz, Matejki Street 21/23 - open 24 hours a day. You do not need to book a place in the dormitory. All candidates who pay for the course automatically have a place reserved in the dormitory.
